REGISTRAR INTERVIEW FORM

this form is to be used by Body Registrars when persons interviewed did not immediately enroll onto service. It is not necessary to fill in this form when a person signs up, pays in full and routes onto service. The purpose of the Reg Interview Form is to provide data on individuals interviewed so that these persons may be kept in contact with and channeled onto service.
