SELECTION ROUTING FORM

FSM Selection Routing Form for routing documents for FSM commission payments. The Body Registrar initiates a Selection Routing Form, which is a document routing form, not a body routing form. A separate form is used for each service signed up. The form has spaces to be filled in by the Body Registrar as to person's name, service signed up for, hour and date of sign up, name of FSM who selected the person for that service, date service is to start, and initials of the Body Registrar attesting to these data.
